Rules for "Captured Moments" Photo Contest

1. A $5.00 entry fee must be included with a recent photo of a child between birth and the age of 10
2. Photographs will not be returned to you due to the large number of entries.
3. Parent(s) or Legal Guardian(s) of a contest winner authorize(s) any photographs received may be published on The Mom & Caregiver family of websites (www.themomonline.com) and/or included in The Mom & Caregiver magazine. Submission of a photograph will be deemed to constitute confirmation and consent of the right to publish the photo without further advice and/or notice.
4. Amateur photographs only please. Professional photographs will not be accepted.
5. Maximum image size should not exceed 8 x 10 inches. A standard smaller size print from any one-hour photographic lab is preferred.
6. On the back of the photograph, please detail: Child's name (only first names will be used to protect your privacy), date of birth, contact details of parent/guardian, approximate date photo was taken (must be within 1 months) and any other little item of interest about your child (which may also be published).
7. $5 entry per photograph; with respect to twins: You may submit one photo with both (or all) the children in it.
8. The selection process will be conducted fairly by a panel of judges with no discrimination towards sex, race, or special needs. The judges will narrow the entries down to the top 40. Finalists will be posted on capturedmoments.ca for online voting to determine the top 13.
9. Parent(s) or Legal Guardian(s) of a contest winner authorize The Mom & Caregiver and Photographer to photograph their child for the purpose of using one of the photographs for the front cover of The Mom & Caregiver Magazine. The staff of The Mom & Caregiver Magazine will choose the final photograph. All decisions are final.
10. The Mom & Caregiver staff and the judges are not eligible to participate in any contest.
11. Parent(s) or Legal Guardian(s) affirm that ownership of submitted photographs have not been granted to any other company or individual.
12. All entries may be mailed in to: The Mom & Caregiver, P.O. Box 29118, 785 Wonderland Rd. S. London, ON N6K 4L9; or may be dropped off at: Cheeky Monkey (Oxford St or Wellington Rd locations) or Westmount Shopping Centre in a sealed envelope along with a signed entry form and $5 fee (cheque or money order payable to LCRN).
13. All entries must be post-marked by January 15, 2012, entries received past this date will not be considered for the contest.
14. All photographs of contest winners become the exclusive property of The Mom & Caregiver and cannot be replicated or copied by any other company or individual without consent from The Mom & Caregiver. Photographs will not be sold to any companies and/or individuals for profit. Photographs will not be provided to any companies and/or individuals for use.
15. Winners must be available for photograph session between March 1st – 15th, 2012 at a predetermined location.
